From Approval to Adherence: Follow-Up Strategies That Keep Patients on Track

Getting a prior authorization approved for a biologic is a big win—but it’s only the first step in a patient’s journey. Too often, patients experience delays in starting their therapy or fall off treatment altogether due to logistical, emotional, or financial challenges. Biologic coordinators play a critical role in bridging that gap, ensuring that an approval turns into consistent, successful treatment. Without this guidance, the time, energy, and resources that went into securing approval can be lost if patients never start or fail to stay on therapy. 
Create a Follow-Up Timeline One of the most effective ways to ensure adherence is to create a structured follow-up plan. Instead of leaving next steps to chance, coordinators can set predictable touch points that anticipate when challenges are most likely to arise. For example: First Dose Check-In: Within 1–2 weeks of approval, reach out to confirm that the medication has shipped, the patient understands how to administer it, and that any immediate concerns are addressed. This initial touchpoint can prevent delays if a pharmacy shipment was held up or if the patient is hesitant to start. Sometimes, simply walking through what to expect during the first injection can reduce a lot of fear.
Three- to Four-Month Milestone: By this point, patients often encounter refill challenges, new side effects, or questions about whether the drug is “working.” Coordinators can check in to verify adherence, help navigate pharmacy processes, and remind patients about labs or follow-ups. Reinforcing the importance of staying consistent—even when symptoms begin to improve—can make the difference between long-term success and early discontinuation. 
Annual Renewal Prep: Begin outreach 30–60 days before authorization expiration. This ensures that new paperwork, updated labs, and insurance verification are completed before the current approval runs out. Proactive renewal also prevents unnecessary treatment gaps, which can be frustrating for both patients and providers.
Proactively Address Barriers Biologic therapy can be intimidating. Patients may struggle with cost, side effects, or fear of self-injection. Coordinators can: Offer injection training via in-office demonstrations, manufacturer nurse support, or video resources. Proactively enroll patients in copay cards, free drug programs, or foundation assistance so that affordability doesn’t lead to abandonment. Normalize side effect discussions. Remind patients to communicate with their care team early instead of stopping therapy on their own. By addressing these barriers up front, coordinators not only support adherence but also strengthen patient confidence in the treatment plan. 
Leverage Tools for Engagement Modern patient engagement strategies combine technology with human connection. Coordinators can: Send text reminders, app-based trackers, or EHR messages to help prevent missed doses. Provide educational handouts with simplified instructions to reduce confusion and empower patients. Celebrate small wins—like the first injection, a successful lab result, or one-year anniversary—which can reinforce that every step forward matters. 
The Emotional Component of Adherence Practical barriers aren’t the only obstacles patients face. Starting a biologic can feel overwhelming, both emotionally and socially. Patients often wonder if needing such a strong medication means their disease is “too severe.” Coordinators can reframe this thinking, reminding patients that starting therapy is a proactive step toward wellness, not a setback. Encouraging patients to share their journey with supportive family members or peer groups can reduce isolation and help them stay motivated. 
The Bigger Picture: Outcomes and Costs 
When patients discontinue therapy prematurely, they often experience worsening disease, higher long-term costs, and a cycle of treatment frustration. Coordinators who take a proactive, structured approach not only improve outcomes but also reduce avoidable denials, refill delays, and patient dissatisfaction. From approval to adherence, every touchpoint matters. A strong adherence program benefits patients, providers, and the healthcare system as a whole.
